Any medical documentation that clearly exhibits your child’s disability will be extremely helpful during your interview. Evidence that comes from a medical professional will boost your case. This may include:
{Check Box} Documents with names, addresses and phone numbers for every doctor or therapist that has treated your child within the past year.
{Check Box} Any important medical records for your child and their patient ID numbers from doctor’s office visits and/or hospitalizations, if they’re accessible.
{Check Box} Any medications your child takes.
{Check Box} Your child’s medical assistance number, if they have one.
While medical records are crucial, there are some other pieces of documentation that can help support your claim. These can include:
{Check Box} Names, addresses and phone numbers of teachers, counselors and therapists your child has seen in the past year.
{Check Box} Contact information for any social service programs or caseworkers who have worked with your child.
{Check Box} Any records from the child’s Individualized Family Service Plan (IFSP) for early intervention services or Individualized Education Program (IEP) for special education services.
{Check Box} Contact information for any caretakers that take care of your child.
{Check Box} The original or certified copy of your child’s birth certificate.
{Check Box} Proof of U.S. citizenship or legal residency, if your child was born outside the United States.
{Check Box} Social Security Numbers for all the people who live in your household.
{Check Box} Proof of income for the household (like pay stubs, self-employment tax returns, unemployment, child support, etc.).
{Check Box} Proof of resources for the household such as bank account statements, life insurance policies, certificates of deposit, stocks or bonds.
